El ensayo clínico controlado y aleatorizado es hoy el método científico hegemónico para evaluar cualquier nuevo procedimiento de diagnóstico o tratamiento médico. Pero su rotundo éxito contrasta con su extrema juventud, en términos históricos: nació hace poco más de medio siglo. Y, para el autor de esta obra singular, el ensayo clínico es en realidad la culminación de un proceso general que se inició tras la Revolución Francesa: la introducción de nuevas técnicas de exploración clínica de enfermos, de métodos cuantitativos y del uso sistemático de estadísticas que convirtieron los hospitales de París, ya en las primeras décadas del siglo XIX, en la luminaria de la medicina mundial. No faltaron las polémicas sobre «la deshumanización de la medicina» por la metodología científica cuantitativa. No faltaron las críticas a los riesgos que para el enfermo concreto supondría la mentalidad probabilística. Polémicas y críticas cuya revisión histórica por Matthews es profundamente iluminadora de los debates actuales entre hechos y valores, entre pruebas y narraciones, entre ciencia objetiva y biografía subjetiva. Pero a pesar de las críticas más o menos fundadas, el cálculo de probabilidades y los métodos cuantitativos se consolidaron en los ámbitos académicos y administrativos del sistema sanitario; la utilización de la estadística se consagró en los estudios de salud pública y de biometría; la aspiración de la ciencia médica a la certeza quedó definitivamente encuadrada en las matemáticas. Como consecuencia de ello, a partir de 1946, los ensayos clínicos asumieron el papel científico y social determinante que juegan en la medicina actual. Esta obra explica brillantemente el cómo y el porqué.
